10 Sneaky Signs Your Gut Might Be Out of Balance (And What to Do About It)

You don’t have to be doubled over with stomach pain or running to the bathroom 10 times a day to have gut issues. The truth is, gut imbalances can show up in all sorts of ways—some of them have nothing to do with your digestion at all.

As a dietitian who works with women on inflammation, mood, and energy, I see it all the time. That nagging bloat, the stubborn skin flare-ups, the fatigue that just won’t quit—it’s often your gut waving a little red flag saying, “Hey! Something’s off in here.”

Let’s break down 10 common signs your gut might be out of balance—and what you can do to feel like yourself again.

1. You’re Always Bloated (Even When You Eat “Healthy”)

Bloating might be the most common gut complaint, and it’s not just annoying—it’s a sign that your digestive system is having trouble breaking down or absorbing food. This could be due to low stomach acid, sluggish motility, bacterial overgrowth, or imbalances in your gut microbiome.

If you feel like everything makes you bloat, it’s time to take a closer look at your gut.

2. You’re Dealing With Constipation or Loose Stools

Your Poop is one of the best clues to your internal health (sorry, not sorry for the poop take, it’s super important). If you’re not going regularly, or your stool is too loose or too hard, it’s a sign that digestion isn’t working properly. And FYI it’s NOT normal to only go every other or every 2 days…you should be going at least once a day!

Chronic constipation can be a red flag for low fiber, dehydration, dysbiosis, sluggish motility, or even gut-brain dysregulation. On the flip side, frequent diarrhea can mean inflammation, food sensitivities, or imbalanced gut bacteria (dysbiosis).

3. You Feel Tired No Matter How Much You Sleep

If you’re dragging yourself through the day, it could be more than just a lack of caffeine. Gut imbalances (like dysbiosis or leaky gut) can impair nutrient absorption—especially B vitamins, iron, and magnesium—leading to low energy, brain fog, and burnout.

Your gut is also where a lot of serotonin and other neurotransmitters are made. When your gut’s off, your brain and mood usually are, too.

4. You’ve Got Brain Fog or Mood Swings

Can’t concentrate? Feeling anxious or irritable for no reason? Your gut might be part of the problem.

The gut-brain axis is a real thing. When the microbiome is disrupted, it can lead to increased inflammation and reduced production of feel-good neurotransmitters like serotonin and GABA. That means more stress, more anxiety, and a shorter fuse.

5. Your Skin Is Breaking Out or Flare-Prone

Eczema, acne, rosacea, skin issues can all be tied back to gut health. When the gut lining is compromised (aka “leaky gut”), inflammatory compounds can sneak into the bloodstream and trigger immune responses that show up on your skin.

This is especially common when there’s an imbalance in gut bacteria or if your body isn’t eliminating toxins effectively.

6. Your Joints Ache or You Feel Puffy

That low-level, lingering inflammation that makes you feel achy, stiff, or swollen? Gut health plays a huge role here, too.

Roughly 70% of your immune system lives in your gut, so when your microbiome is off, it can trigger chronic systemic inflammation—hello joint pain, water retention, and even autoimmunity.

7. You React to More Foods Than You Used To

Do you feel like you’re suddenly sensitive to everything? If your list of “safe foods” keeps shrinking, your gut may be inflamed or overly permeable.

When the gut lining is compromised, larger food particles can pass through, triggering an immune response. That leads to food sensitivities, bloating, and sometimes full-on histamine reactions.

8. You Get Sick Easily or Can’t Seem to Kick That Cold

If your immune system feels like it’s constantly playing defense, your gut might be struggling. As mentioned, the gut is a major immune hub, and a balanced microbiome helps regulate immune function.

If you’re dealing with frequent colds, flus, or lingering infections, your gut flora might need some love.

9. You Have Intense Sugar or Carb Cravings

Certain bacteria in your gut actually feed on sugar, and the more of those microbes you have, the more they scream for sugar.

This creates a feedback loop: sugar feeds the “bad” bugs, and those bugs make you crave even more sugar. Breaking that cycle takes targeted nutrition and a microbiome reset.

10. You’ve Had a Lot of Stress, Antibiotics, or other medication

Okay, this one’s more of a cause than a symptom, but it matters. Some Medication can wipe out beneficial gut bacteria, disrupt digestion, and set the stage for long-term imbalances.

Even if you’re not having dramatic symptoms now, a history of antibiotics or chronic stress can silently set the gut up for trouble later.

So… What Do You Do About It?

If you’ve read this far and thought, Yep. That’s me. The good news is that your gut is incredibly resilient. With the right support, you can feel better. Like way better.

Here’s where to start:

1. Remove the Triggers

Identify and reduce common gut disruptors:

  • Ultra-processed foods
  • Excess sugar
  • Alcohol
  • Food sensitivities
  • Environmental toxins
  • Chronic stress
  • Dysbiosis

This is the “Remove” phase of gut healing—clearing out what’s causing the inflammation or feeding imbalances.

2. Support Digestion

Add back the things your gut may be missing:

  • Digestive enzymes or bitters
  • Stomach acid support (like lemon water or ACV before meals)
  • Mindful eating practices (yes, chewing thoroughly counts!)

3. Reinoculate With Good Bacteria

This means repopulating your gut with beneficial bacteria through:

  • Fermented foods (sauerkraut, kimchi, yogurt)
  • Prebiotic fibers (like garlic, onions, asparagus, green bananas)
  • High-quality probiotics (strain-specific if possible)

4. Repair the Gut Lining

Support gut integrity with nutrients like:

5. Regulate the Nervous System

Your gut can’t heal in a constant state of stress. Tools like breathwork, somatic movement, vagus nerve support, and even laughter are powerful medicine.

This is where we connect the dots between the gut and the rest of your life. Because healing isn’t just about what’s on your plate—it’s about how you live.
